DOCUMENTS AND DETAILS REQUIRED TO OPEN A CDS A/C FOR NON-RESIDENT FOREIGNERS & NON RESIDENT SRI LANKANS

Complete the documents given under A or B depending on the residence status.

A – NON RESIDENT FOREIGNERS (Attached documents 1-3 to be completed)

1. Proof of Residence – A copy of any utility bill or bank statement or such other proof which is not more than 3 months old. (Eg: Electricity, Water, Telephone bills etc.)
   
   
2. Securities Investment Account (SIA) details with documentary evidence. (SIERA has been replaced with SIA)
   
   
3. A copy of valid foreign passport

Please Note:

• All supporting documents should be certified as true copies of originals by a solicitor, Attorney –at –law (relevant country) or an embassy official.
  
  
• When filling the application forms please avoid using correction fluid.
  
  
• Applicant’s signature has to be witnessed by two persons.
  
  
• If the account is to be opened using Power of Attorney (POA), a copy of a valid POA is required.
  
  
• An SIA account should be opened prior to opening a CDS account
  
  
• If a non national applicants wishes to open an account giving a local correspondence address, she/he should provide sufficient information regarding the correspondence address ( Eg : Valid residential visa )
  
  
• If the applicant has dual citizenship, she/he has to complete documents given in under "A" ( Non Resident Foreigners).
