2. Know Your Customer Form (KYC)
   
   
3. Securities Investment Account (SIA) details with documentary evidence
   
   
4. i. Name/s, Address, Occupation of Directors
   ii. Passport copies of directors
   
   
   • If the Director/s is/are also Company/ies, the following information on such Director Company should be given;
     1. Name of the Company
     2. Date of Incorporation
     3. Place of Incorporation
     4. Registered Address
        
        
   • If an authorized person signs the CDS application form on behalf of the applicant company requires
   1. A copy of the Board Resolution/Power of Attorney authorising such person to sign on behalf of the company
   2. Passport copy/ies of such persons.

    

5. Name and address of top 10 shareholders
   
   
6. Name of person/s authorized to give instructions with a copy of the Power of Attorney/Board Resolution.
   
   
7. Following documents are required.
   1. Certified copies of Memorandum & Articles Association
   2. Certified copy of Certificate of Incorporation
   3. Board Resolution (Part of account opening)
   4. Certified copy of Certificate to Commence Business (where relevant)
   5. Certified copy of “Good Standing” issued by the Registrar of
   6. Companies/applicable authority where the company is incorporated

• All supporting documents should be certified as true copies of originals by a solicitor, Attorney –at –law (relevant country) or an embassy official.
