2. Know Your Customer Form (KYC)
   
   
3. a. Name/s, Address, occupation of directors
   b. Certified copy of NIC or Passport of Directors (valid passport copy)
   (NIC / Passport copy should be certified by an attorney-at –law / Notary Public)
   
   
4. Name and address of top 10 shareholders
   
   
5. Certified copies of Memorandum & Articles of Association
   
   
6. Certified copy of Certificate of Incorporation
   
   
7. Board Resolution (Only if the 'Certificate of Resolution' on account opening form is not filled)
   
   
8. If an authorized person signs the CDS application form on behalf of the applicant company, a copy of NIC or Passport of the authorized person should be forwarded to us. (Place the embossed seal if using)
   
   
9. Name of person/s authorized to give instructions.

Please Note:

• When filling the application forms please avoid using correction fluid.

• All supporting documents should be certified as true copies of the originals, by a Solicitor/Attorney –at –law

• Details required in 3 (a) and 4 above have to be provided on a company letterhead duly certified by the company secretary or director.

• Documents mentioned in 4, 5, 6 above are not required if it is a listed company.
